Criteria for Admission to Hawai'i Island Home for Recovery, Inc.'s Program Services
Participant Admission Criteria

Must be a voluntary, consenting adult who is eighteen (18) years or older.

Must be at a level of care that does not require constant monitoring.

Must be referred via Bridging the Gap's Coordinated Entry System*

The applicant will be interviewed by the Executive Director & Staff to verify program eligibility.

The applicant will complete HIHR's application and intake forms.

The applicant will provide a photocopy of the following documents:

Government issued identification
-
Social Security Card
-
Health Insurance Card(s)
-
Proof of Income (pay statements, invoices, etc.)
-
TB test results, Hepatitis, or any other transmitted disease results as requested.

Negative COVID-19 test.

Exclusion Criteria
-
There are noted displays of assaultive/aggressive behaviors by the applicant at the time of referral or within the last six (6) months.

Applicants who are not at-risk for homelessness.

Applicants who do not voluntarily consent to comply with Hawai'i Island Home for Recovery, Inc. HIHR Policies and Rules

Children/Adolescents are not eligible for HIHR's services
